Incident Analysis

DP Station Keeping Incidents

IMCA (and its forerunner, the Dynamic Positioning Vessel Owners Association - DPVOA) has been collecting information on DP incidents, and compiling annual reports which list and analyse them, since 1990. Data held extends back a further ten years.

In addition, one of the latest products of the Marine Division is a database, available on CD-ROM to members only as IMCA M 156, which brings together the data contained in the annual reports 1990-99, along with that from IMCA M 150 - "Quantified Frequency of Shuttle Tanker Collision during Offtake Operations". Newly updated, the database extends to over 500 incidents over the period, with extensive analysis and fault trees.

Both in printed and electronic form, IMCA provides access to (and, for members, annual updates to) this extensive array of data, widely acclaimed globally across the industry, which forms a major resource for helping the industry to prevent repeat incidents. The summary information gained from the annual analysis also provides useful input to assist the Marine Division in developing a technical work programme to address the technical issues facing specialist vessel operators.

DP Incident Reporting Forms - The IMCA DP incident reporting forms were fully revised and updated in January 2006 to reflect new reporting categories aimed at simplifying decisions on incident classification, enabling more in-depth analysis and encouraging reporting of incidents. The new forms should be used with immediate effect and all members are asked to replace any older versions they may have (which in some cases carry out-of-date return contact details). Lifting Incidents

IMCA is establishing a scheme, similar to that for DP incidents, to cover data collection for analysis of lifting incidents. The reporting form has been reviewed and updated in 2005 to make it more user-friendly, requiring less time to complete and with useful guidance attached to the form. The industry is being encouraged to make this small additional effort when incidents occur to help prevent them happening in the future.
