This joint safety alert (attached) has been issued by the Bureau of Safety and Environment Enforcement (BSEE) and the United States Coastguard (USCG). It addresses a dynamic positioning (DP) incident involving an Offshore Supply Vessel (OSV) which resulted in a loss of position during a critical activity attached to a wellhead.
