The Marine Safety Forum has published Safety Flash 11-02 regarding an incident in which, owing to a failure of communication, attempts were made to lift equipment which was still secured to the deck.
Although no harm was done, the incident could have resulted in serious injury to personnel and damage to the equipment being lifted.
