The Marine Safety Forum has published Safety Flash 13-10 covering an incident in which a crew member was injured when a trolley and chain hoist came off the lifting beam.
The immediate cause of the incident was that in certain circumstances there was no end stopper to prevent the trolley and chain host from coming off the end of the beam.
